What is Nulojix?

Nulojix is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Bristol-Myers Squibb Company.

Nulojix is a selective T-cell costimulation blocker indicated for prophylaxis of organ rejection in adult kidney transplant recipients. It is used in combination with basiliximab induction, mycophenolate mofetil, and corticosteroids. Only use in patients who are EBV seropositive. Use has not been established for the prophylaxis of organ rejection in transplanted organs other than the kidney.

Increased risk for developing post-transplant lymphoproliferative disorder (PTLD), mainly involving the central nervous system (CNS). Patients without immunity to Epstein-Barr virus (EBV) are at increased risk therefore, use in EBV seropositive patients only. Do not use in transplant recipients who are EBV seronegative or with unknown serostatus. Only physicians experienced in immunosuppressive therapy and management of kidney transplant patients should prescribe Nulojix. Increased susceptibility to infection and the possible development of malignancies may result from immunosuppression. Use in liver transplant patients is not recommended due to an increased risk of graft loss and death.

What doses of Nulojix are available?

Nulojix is available in lyophilized powder for injection form, in the following doses: 250 mg per vial.

Dosing for Initial Phase:

  • Day 1 (day of transplantation, prior to implantation) and Day 5 (approximately 96 hours after Day 1 dose) 10 mg per kg
  • End of Week 2 and Week 4 after transplantation – 10 mg per kg
  • End of Week 8 and Week 12 after transplantation – 10 mg per kg

Dosing for Maintenance Phase:

  • End of Week 16 after transplantation and every 4 weeks (plus or minus 3 days) thereafter – 5 mg per kg.

For intravenous infusion only, administer over 30 minutes. Only use the enclosed silicone-free disposable syringe to prepare for administration.

Nulojix active ingredients

The active ingredient of Nulojix is belatacept, which weakens your body’s immune system, to stop it from “rejecting” a transplanted organ such as a kidney.

Nulojix side effects

The most common side effects of Nulojix in clinical trials include:

  • Anemia, hypokalemia, hyperkalemia, and leukopenia (low white blood cells)
  • Diarrhea, constipation
  • Nausea, vomiting, headache, cough
  • Urinary tract infection
  • Peripheral edema
  • Hypertension (high blood pressure)
  • Pyrexia
  • Graft dysfunction

In rare instances, Nulojix can cause more serious adverse reactions. These can include:

  • Post-Transplant Lymphoproliferative Disorder (PTLD)- see Boxed Warning
  • Other malignancies
  • Progressive Multifocal Leukoencephalopathy (PML)
  • Other serious infections – increased risk of bacterial, viral, fungal, and protozoal infections, including opportunistic infections and tuberculosis. Polyoma virus-associated nephropathy can lead to kidney graft loss
  • Acute rejection and graft loss with corticosteroid minimization
  • Coadministration with anti-thymocyte globulin in de novo kidney transplant recipients, especially those with other risk factors for venous thrombosis of the renal allograft, coadministration with anti-thymocyte globulin may pose a risk for venous thrombosis of the renal allograft
  • Risk of rejection with the conversion from a CNI based maintenance regimen – conversion of maintenance kidney transplant recipients from a CNI based to Nulojix based maintenance regimen increases the risk of acute rejection. Conversion of stable kidney transplant recipients from a CNI based maintenance therapy to belatacept-based maintenance therapy is not recommended unless the patient is CNI intolerant

Nulojix drug interactions

Nulojix can interact with other medications. These include:

  • Mycophenolate mofetil (MMF) – when patients are switched between cyclosporine and Nulojix therapy
  • Immunizations – avoid the use of live vaccines during treatment

This list is not exhaustive and other prescription drugs may interact with Nulojix.

Nulojix contraindications

You should not use Nulojix if you:

  • Are allergic to the active ingredient belatacept
  • Are allergic to any of the other ingredients in Nulojix
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Talk to your doctor before using Nulojix if you:

  • Are taking any of the medications that could interact with Nulojix
  • Have other known risk factors for PTLD include cytomegalovirus (CMV) infection and T cell-depleting therapy

Nulojix generic

Nulojix is the trademark brand name for belatacept manufactured by Bristol-Myers Squibb Company. A generic version of belatacept is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Nulojix savings through NiceRx.
