What is Lomotil?

Lomotil is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Greenstone LLC. Lomotil tablets contain 2.5 mg of diphenoxylate hydrochloride and 0.025 mg of atropine sulfate.

Atropine is classed as an anticholinergic and helps to reduce spasms in the bladder, stomach, and intestines. Diphenoxylate is an antidiarrheal medication. Lomotil is used to treat diarrhea and to decrease the number and frequency of bowel movements.

Lomotil is used in the management of diarrhea in patients 13 years of age and older. It is not known if Lomotil is safe and effective in children younger than 2 years of age.

Lomotil is classed as a Schedule V controlled substance by federal regulation. Diphenoxylate hydrochloride is chemically related to the narcotic analgesic meperidine. In doses used for the treatment of diarrhea, whether acute or chronic, diphenoxylate has not produced addiction. Diphenoxylate hydrochloride does not have morphine-like subjective effects at therapeutic doses, but at high doses, it exhibits codeine-like subjective effects. The dose which produces an antidiarrheal effect is widely separated from the dose which causes central nervous system effects.

What doses of Lomotil are available?

Lomotil is available in tablet form, in the following doses: 2.5 mg of diphenoxylate hydrochloride and 0.025 mg of atropine sulfate.

You should drink plenty of liquids to keep from getting dehydrated while you have diarrhea. Your doctor may recommend an electrolyte supplement while you are taking Lomotil.

Store Lomotil tablets at room temperature away from moisture and heat, where a child cannot reach it. An overdose of atropine and diphenoxylate can be fatal to a child.

Lomotil contains diphenoxylate hydrochloride and atropine sulfate as the active ingredient.

Lomotil side effects

The most common side effects of Lomotil in clinical trials include:

  • Drowsiness, dizziness
  • Headache
  • Numbness in your hands or feet
  • Depression
  • Confusion
  • Red or swollen gums
  • Dry mouth, nose, or throat
  • Nausea, vomiting, upset stomach, loss of appetite
  • Skin rash, dryness, or itching

In rare instances, Lomotil can cause more serious side effects. These can include:

  • Severe constipation, stomach pain, or bloating
  • Ongoing or worsening diarrhea
  • Severe pain in your upper stomach spreading to your back
  • Fever
  • Hallucinations, seizure
  • Rapid breathing, weak or shallow breathing
  • Fast heart rate
  • Dehydration symptoms–feeling very thirsty or hot, being unable to urinate, heavy sweating, or hot and dry skin.

Lomotil drug interactions

Lomotil can interact with other medications. These include:

  • Alcohol
  • Other drugs that cause CNS depression – barbiturates, benzodiazepines, opioids, buspirone, antihistamines, muscle relaxants
  • Monoamine oxidase inhibitors (MAO) inhibitors – diphenoxylate may interact with (MAOIs) to cause a hypertensive crisis (headache, hyperthermia, hypertension)

This list is not exhaustive and other prescription drugs may interact with Lomotil. Talk with your doctor and pharmacist. Tell them about all prescription drugs, over-the-counter, and other drugs you take. Also tell them about any vitamins, herbs, and supplements you use.

Lomotil contraindications

You should not use Lomotil if you:

  • Are allergic to the active ingredient atropine and diphenoxylate
  • Have had an allergic reaction to any of the other ingredients in Lomotil
  • Have diarrhea that is caused by bacteria or by taking an antibiotic
  • Have obstructive jaundice

Talk to your doctor about your medical condition before using Lomotil if you:

  • Are taking any of the medications that could interact with Lomotil
  • Have a blockage in your intestines
  • Have ulcerative colitis
  • Have asthma or other breathing problems
  • Have glaucoma
  • Have urination problems
  • Have liver or kidney disease
  • Have Down’s syndrome
  • Are dehydrated
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed – this medication passes into breast milk
