What is Lokelma?

Lokelma is an FDA-approved medication manufactured by AstraZeneca. The FDA approval is supported by data from three double-blind, placebo-controlled trials and two open-label trials in adult patients with hyperkalemia. Lokelma is used for the treatment of hyperkalemia (high levels of potassium) in adults. Hyperkalemia is associated with cardiovascular, renal, and metabolic diseases. Lokelma is not absorbed into your bloodstream. It acts directly on the intestines. Lokelma has a delayed onset of action and should not be used for life-threatening hyperkalemia treatment in emergencies.

Lokelma powder needs to be mixed with water before drinking. All of the powder from the packet needs to be used to make sure the correct dose is taken. The dose of Lokelma will need to be adjusted depending on your body serum levels.

Other medicines should be taken 2 hours before or 2 hours after you take this medicine as Lokelma can make it harder for your body to absorb other medicines you take by mouth. Patients receiving chronic hemodialysis should take Lokelma only on non-dialysis days. The recommended starting dose is 5 g once daily on non-dialysis days.

What doses of Lokelma are available?

Lokelma is available in powder form, for oral suspension, in 5 g doses. Each 5 g dose contains approximately 400 mg of sodium.

Lokelma active ingredients

Lokelma contains the prescription medicine sodium zirconium cyclosilicate. Lokelma is a strong potassium binder and works by replacing hydrogen and sodium for the excess potassium in the body. Lokelma’s mechanism of action is to increase the excretion of potassium in stools from your body by binding with the potassium in the gastrointestinal tract. This mopping up of the excess potassium helps to reduce the amount of potassium that is free in the gastrointestinal tract and therefore helps to lower your serum potassium levels.

Lokelma side effects

The most common side effects and adverse reactions caused by Lokelma included edema (swelling) and hypokalemia (low potassium levels).

Other less common side effects of Lokelma include:

  • Nausea (feeling sick)
  • Vomiting (being sick)
  • Irregular heartbeat
  • Dry mouth, increased thirst, decreased urinating
  • Seizures
  • Feeling tired or weak
  • Loss of appetite
  • Mood changes
  • Muscle pain or cramps, tingling or numb in hands, feet, and lips

Lokelma drug interactions

Lokelma can interact with other prescription drugs. These include:

  • Medication to lower your cholesterol – atorvastatin
  • Antifungal medication – itraconazole, ketoconazole, and posaconazole
  • Diuretics such as furosemide
  • Antiviral medication – atazanavir, indinavir, nelfinavir, ritinovir, and saquinavir
  • Medications taken to treat certain types of cancer – dasatinib, erlotinib, and nilotinib
  • Anticoagulants taken to prevent blood clotting – dabigatran

This list is not exhaustive and other drugs may interact with Lokelma.

Lokelma contraindications

You should not use Lokelma if you:

  • Are allergic to the active ingredient sodium zirconium cyclosilicate
  • Are allergic to any of the other ingredients in Lokelma

Talk to your doctor before using Lokelma if you:

  • Are taking any of the medications that could interact with Lokelma
  • Have heart failure or chronic kidney disease (CKD) – hyperkalemia is more likely in patients taking drugs that inhibit the renin-angiotensin-aldosterone system (RAAS)
  • Are on a low-salt diet
  • Have a bowel obstruction, slow digestion, or severe constipation
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Lokelma generic

Lokelma is the trademark brand name for sodium zirconium cyclosilicate manufactured by AstraZeneca. A generic version of Lokelma is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Lokelma savings through NiceRx.
