If you are successfully enrolled in the Lazanda patient assistance program, we will also request your Lazanda medication refills on your behalf for up to 1 year. At the end of your enrollment period, we will contact you for updated details and seek to re-enroll you in the program.

What is Lazanda?

Lazanda is an FDA (U.S. Food and Drug Administration) approved medication. Lazanda is an opioid agonist for the management of breakthrough cancer pain in patients 18 years of age and older who are already receiving and who are opioid-tolerant to opioid therapy for their underlying persistent cancer pain.

  • Due to the risk of fatal respiratory depression, Lazanda is contraindicated in opioid non-tolerant patients and in the management of acute or postoperative pain, including headache/migraines
  • Keep out of reach of children
  • Use with CYP3A4 inhibitors may cause fatal respiratory depression
  • When prescribing, do not convert patients on an mcg per mcg basis from any other oral transmucosal fentanyl product to Lazanda. This may result in a fatal overdose
  • When dispensing, do not substitute with any other fentanyl products
  • Contains fentanyl, a Schedule II controlled substance with an abuse liability similar to other opioid analgesics such as hydromorphone, methadone, morphine, and oxycodone
  • Lazanda is available only through a restricted program called the TIRF REMS Access program. Outpatients, healthcare professionals who prescribe to outpatients, pharmacies, and distributors are required to enroll in the program

What doses of Lazanda are available?

Lazanda is available as a nasal spray, in the following doses: each spray delivers 100 mcL of the solution containing either 100 mcg or 400 mcg fentanyl base. Supplied in a 5 mL bottle containing 8 sprays.

Patients must require and use around-the-clock opioids when taking Lazanda. The initial dose of Lazanda for all patients is 100 mcg. Individually titrate patients to an effective dose, from 100 mcg to 200 mcg to 400 mcg, and up to a maximum of 800 mcg, that provides adequate analgesia with tolerable side effects. The dose is a single spray into one nostril or a single spray into each nostril (2 sprays). The maximum dose is a single spray into one nostril or a single spray into each nostril per episode, and no more than four doses per 24 hours. Wait at least 2 hours before treating another episode of breakthrough pain with Lazanda.

During an episode, if adequate pain relief is not achieved within 30 minutes, the patient may use a rescue medication as directed by their healthcare provider. Always speak with your healthcare provider about any changes to your dose so they can monitor and evaluate your condition.

Lazanda active ingredients

Lazanda (fentanyl nasal spray) contains a liquid formulation of fentanyl citrate for intranasal transmucosal administration.

Lazanda side effects

The most common side effects of Lazanda in clinical trials include:

  • Vomiting, nausea, dizziness
  • Pyrexia
  • Constipation

In rare instances, Lazanda can cause more serious adverse events. These can include:

  • Clinically significant respiratory and CNS depression can occur
  • Physical dependence results in withdrawal symptoms in patients who abruptly discontinue the drug

Lazanda drug interactions

Lazanda can interact with other medications. These include:

  • Use with other Central Nervous System CNS depressants and potent CYP3A4 inhibitors (erythromycin, ketoconazole, ritonavir, grapefruit juice) may increase depressant effects including hypoventilation, hypotension (low blood pressure), and profound sedation
  • CYP3A4 inducers – rifampin, carbamazepine, phenytoin
  • Benzodiazepines and other sedatives, anxiolytics, tranquilizers, muscle relaxants, general anesthetics, antipsychotics, other opioids, alcohol
  • Serotonergic drugs
  • Tricyclic antidepressants
  • Monoamine oxidase (MAO) inhibitors – phenelzine, tranylcypromine, linezolid
  • Mixed agonist/antagonist and partial agonist opioid analgesics – pentazocine, buprenorphine
  • Concomitant use of vasoconstrictive nasal agents to treat allergic rhinitis can compromise the efficacy of Lazanda

This list is not exhaustive and other prescription drugs may interact with Lazanda. Tell your doctor about all the medicines you take, including prescription drugs and nonprescription medicines, vitamins, and herbal supplements.

Lazanda contraindications

You should not use Lazanda if you:

  • Are allergic to the active ingredient fentanyl
  • Have an intolerance or hypersensitivity to fentanyl or components of Lazanda
  • Are taking medication for the management of acute or postoperative pain including headache/migraine or dental pain

Talk to your doctor before using Lazanda if you:

  • Are taking any of the medications that could interact with Lazanda
  • Have a renal or hepatic impairment
  • Have breathing problems or sleep apnea
  • Have a head injury, brain tumor, or suffer from seizures
  • Have a drug or alcohol addiction
  • Have a mental illness
  • Have problems urinating
  • Have a blockage in your stomach or intestines
  • Have problems with your gallbladder, pancreas, or thyroid
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Lazanda generic

Lazanda is the trademark brand name for fentanyl. A generic version of fentanyl is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Lazanda savings through NiceRx.
