How do I apply for the Caduet patient assistance program?

Our process makes it as easy as possible to apply for the Caduet patient assistance program. Begin by completing the enrollment application form on our website. Tell us about any medications that you are taking, including Caduet, your healthcare provider, your insurance plan, and your household income. What is Caduet?

Caduet is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Pfizer. It is a combination of amlodipine besylate, a calcium channel blocker, and atorvastatin calcium, an HMG CoA-reductase inhibitor, indicated in patients for whom treatment with both amlodipine and atorvastatin is appropriate.

Amlodipine is indicated for the treatment of hypertension, to reduce high blood pressure, or prevent chest pain (angina). Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, such as strokes and myocardial infarctions. Amlodipine is indicated for the treatment of Coronary artery disease. It works by relaxing the blood vessels so that blood can flow more easily and the heart does not need to work so hard.

Atorvastatin is indicated for the prevention of cardiovascular disease and hyperlipidemia (high cholesterol). Atorvastatin is classed as a statin and is used to lower the LDL (“bad” cholesterol) and triglycerides and raise “good” cholesterol (HDL) in the blood. This decreases the risk of heart disease and helps prevent strokes and heart attacks. Atorvastatin can also lower the risk of heart attack or stroke in patients with diabetes and risk factors such as diabetic eye or kidney problems, smoking, or high blood pressure.

What doses of Caduet are available?

Caduet is available in tablet form, in the following doses: amlodipine 2.5, 5, or 10 mg and atorvastatin 10, 20, 40, or 80 mg.

Read the prescribing information and patient information and always speak with a healthcare professional for medical advice or about any changes to your dose so they can monitor and evaluate your condition.
