What is Arranon?

Arranon is an FDA (U.S. Food and Drug Administration) approved medication distributed by Novartis Pharmaceuticals.

It is classed as a nucleoside metabolic inhibitor for the treatment of T-cell acute lymphoblastic leukemia and T-cell lymphoblastic lymphoma (T-LBL) in patients who have not responded to or have relapsed after treatment with at least two chemotherapy regimens. This use is on the basis of induction of complete responses.

Severe neurologic adverse reactions have been reported when using this treatment. These include altered mental states such as severe somnolence, convulsions, and peripheral neuropathy. Adverse reactions with demyelination, and ascending peripheral neuropathies similar in appearance to Guillain-Barré syndrome have also been reported.

What doses of Arranon are available?

Arranon is available in injection form, in the following doses: 250 mg/50 mL (5 mg/mL) vial.

Nelarabine injection for adult patients is administered intravenously over 2 hours on days 1, 3, and 5 repeated every 21 days. The pediatric dose is administered intravenously over 1 hour daily for 5 consecutive days and repeated every 21 days.

Monitor for neurologic adverse reactions and discontinue treatment for neurologic adverse reactions of NCI common toxicity criteria grade 2 or greater.

Measures should be taken to stop hyperuricemia such as hydration, urine alkalinization, and prophylaxis with allopurinol. Arranon is not recommended in pediatric patients younger than 1 year of age.

Arranon active ingredients

Arranon contains nelarabine a prodrug of the cytotoxic deoxyguanosine analogue, 9-β-Darabinofuranosylguanine (ara-G). Nelarabine is classed as an antimetabolite. Its mechanism of action is on specific cell cycles and to attack cells at very specific phases in the cycle. Antimetabolites are classed based on the substances with which they interfere. Nelarabine is classed as an adenosine deaminase inhibitor.

Arranon side effects

The most common side effects of Arranon in clinical trials include:

  • Anemia, thrombocytopenia, neutropenia including febrile neutropenia, leukopenia
  • Nausea, diarrhea, vomiting, constipation
  • Fatigue, pyrexia, cough, and dyspnea
  • Neurological adverse reactions include somnolence, dizziness, peripheral neurologic disorders, hypoesthesia, headache, and paresthesia
  • Signs of dehydration such as tiredness, thirst, dry mouth, dark and decreased amount of urine, or dizziness
  • Unusual bleeding or bruising
  • Black or tarry stools, or blood in your stools
  • Blood in the urine
  • Chest pain or pressure or a fast heartbeat

In rare instances, Arranon can cause more serious adverse effects. These can include:

  • Severe neurologic adverse events – extreme sleepiness, somnolence, convulsions, ataxia, confusion, and paresthesias
  • Hematologic reactions – complete blood counts including platelets should be monitored regularly
  • Fetal harm
  • Hematologic
  • Tumor Lysis Syndrome (the rapid breakdown of a large number of cancer cells)

Get medical help if you notice any of the following symptoms of a serious allergic reaction, rash, itching, swelling (especially of the face, tongue, or throat), severe dizziness, and trouble breathing.

Arranon drug interactions

Arranon can interact with other medications. These include:

  • Adenosine deaminase inhibitors, such as pentostatin

Arranon contraindications

You should not use Arranon if you:

  • Are allergic to the active ingredient nelarabine
  • Are allergic to any of the other ingredients in Arranon
  • Have recently been given a live vaccine
  • Are pregnant or are planning to become pregnant

Talk to your doctor before using Arranon if you:

  • Are taking any of the medications that could interact with Arranon
  • Have renal impairment or any other kidney problems
  • Have hepatic impairment – monitor for toxicities
  • Have a nervous system disorder
  • Have had prior chemotherapy or radiation treatment of your head, neck, or spinal cord
  • Are breastfeeding or are planning to breastfeed

Arranon generic

Arranon is the trademark brand name for nelarabine distributed by Novartis Pharmaceuticals. A generic version of nelarabine is available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Arranon savings through NiceRx.
